New Client Forms & Information
New clients, please print out the following forms, take your time to read and complete them, and bring them to your first session: For your information:Self-Help Worksheets
AAA Coping Worksheet - This worksheet guides you through 3 A's for coping: Awareness, Acceptance, and Action. It provides a frameork for applying both acceptance and change approaches to psychological well-being. I highly recommend applying the steps in the order presented. Naturally, people want to skip the first 2 As and go right to "Action." Be patient, implement Awareness and Acceptance first, and your Actions will be much more productive.Mindful Cognitive Processing Worksheet - This worksheet is a special blend of mindfulness and traditional cognitive therapy. Excellent for navigating depressive and anxious episodes. Consult the books, "Mind Over Mood" or "Feeling Good" for more details on cognitive therapy.
Anxiety Self-Monitoring Log and Fear/Avoidance Log - These worksheets are useful for assessing the nature of your anxiety. This is an important first step in a cognitive-behavioral approach to relieving anxiety as presented, for example, in "The Shyness & Social Anxiety Workbook" (Antony & Swinson).
Ten Principles for Cultivating Peace of Mind and Body
I've summarized ten guiding principles, largely based on Zen or Buddhist principles, which I have found universally helpful in developing internal balance. You can see this list to the right and you can read a detailed description of these principles here. I recommend practicing contemplating these principles in many different situations as you navigate through life's twists and turns. To deepen your understanding of these principles, you might enjoy books and recordings by Jack Kornfield, Jon Kabat-Zinn, Tara Brach, and Pema Chodron.Restorative Breathing CD
Guided practices for restoring vitality to the body and mind. I've created this CD to help guide people to practice mindfulness and to develop more balanced breathing to calm the mind and body. In includes one hour of practices which flow together into a continuous session. Each track also stands alone, so that you can practice shorter sessions or mix and match the segments depending on your present needs and time available. This work is mased on mindfulness principles and breathing techniques as taught by Dennis Lewis and Donna Farhi.
